Become A Patient
Please complete the following form prior to your consultation and bring it to your appointment. We ask that you also bring a form of ID, such as your driver’s license, and your insurance card.
If you would like to complete the new patient paperwork electronically prior to your appointment, please click here. This will assist our office as we have transitioned to Electronic Medical Records.
A consultation fee is required at the time of your appointment. If you decide to have a procedure or surgery, the fee will be applied to your balance.
We also accept the following forms of payment: cash, check, and all major credit cards.
Please contact our office 24 hours in advance if you are unable to keep your appointment. If you cancel your appointment less than 24 hours in advance, a $50.00 cancellation fee will be charged. We attempt to accommodate our patients’ schedules to the best of our ability, and by receiving your cancellation prior to 24 hours, we are able to accommodate another patient. Please be advised that if you do not show for two consecutive appointments, another appointment will not be made.
We have special rates with premier hotels close to our facility. These include The Strathallan, Courtyard Marriott, the Inn on Broadway, and the Hyatt Regency. Please contact a Patient Consultant for more information.
For added convenience and comfort following ambulatory surgery, some patients choose to stay in a supervised setting at our Carriage House on their first post-operative night, located adjacent to the Lindsay House. Especially for guests without the benefit of family or friends in the Rochester area, our staff is happy to assist you with overnight arrangements.
We are committed to your privacy. To ensure a comfortable experience for you as our patient, please make use of our private entrance and isolated waiting area located near a side entrance of our facility.
Q VIP Program:
To reward our loyal patients, we are pleased to offer the Q VIP program. Points are earned for every dollar spent on injectables and cosmetic surgery which can then be redeemed for a credit off a future procedure. To learn more, click here.
